Social Worker (BSW)- Mobile Crisis Rapid Response Team- Temporary Full Time

The Sault Area Hospital (SAH) is currently recruiting for a Social Worker for the Mobile Crisis Rapid Response Team.  The successful candidate will be an integral member of the multidisciplinary mental health team and will liaise with the diverse mental health programs within the hospital and community.  There will be a focus on developing strong relationship with police and community mental health and addiction providers to assist in the development and implementation of processes, protocols and pathways to ensure strong support for individuals being served by MCRRT/Crisis Services.    The role will be to transition clients from the acute phase of interactions with MCRRT to the community resources. 

 

DUTIES:

  1. Provide direct social work service, mental health assessments and counseling to clients and/or their families as a part of an interdisciplinary team.
  2. Interview clients individually, in families or in groups to determine and evaluate the client’s social, financial, psychological, cultural and family situation and total needs to provide a comprehensive assessment and treatment plan.
  3. Plan programs of assistance for clients including referral to agencies that provide financial assistance, housing, medical treatment and other interdependent services.
  4. Work in collaboration with Police services toprovide acute crisis intervention in the community.
  5. Serve as a member on interdisciplinary teams of professionals working with client groups.
  6. Provide consultation to members of hospital staff, community agencies and other persons or groups seeking guidance in an effort to address the problems of the clients.
  7. Participate in In-Service programs regarding the social and emotional components of health/illness and their effects on patients and families, as assigned.
  8. Assist individuals in the transition from an inpatient/emergency department or community service to the community.
  9. Assist in diversion from hospital and the criminal justice system as appropriate.
  10. Work to grow strong community partnerships with a focused attention on transitional support.
  11. Work from a continuous improvement framework.
  12. Provide follow up and regular wellness checks proactively or by appropriate request.
  13. Provide ongoing education to SAH, Police services and the community.
  14. Educate clients, families and other supports on mental health and addictions and how to access support.
  15. Maintain accurate client records and statistical reports as required.
  16. Evaluate the effectiveness of counseling and social programs.
  17. Function as a professional role model demonstrating good interpersonal skills and a commitment to self-growth and development.
  18. Develop and maintaining good working relationships with other hospital and medical staff while remaining a client advocate.
  19. Participate in peer supervision with other staff, as requested.
  20. Maintain current knowledge of best practices and benchmarks for the delivery of services, appropriate research, literature, accreditation requirements, policy and legislative requirements and implementing improvements/changes as appropriate.
  21. Comply with organizational policies, procedures and standards.
  22. Develop and maintain client centered treatment plans.
  23. Assuming other duties as may be assigned.

 

QUALIFICATIONS:

  1. Bachelor’s degree in Social Work or other relevant field.
  2. Current registration with the Ontario College of Social Workers or other governing body.
  3. Must possess a valid drivers’ license and the ability to drive a vehicle
  4. Non-Violent Crisis Intervention, and Applied Suicide Intervention Skills Training

 

 

EXPERIENCE:

  1. Minimum 3 years of direct experience in providing mental health or crisis counselling in an acute setting. The experience would need to have been with completing mental health & addiction assessments, interventions and treatment planning for clients in acute distress due to situational crisis, as well as those that have complex Mental Health and Addictions diagnosis and needs. 
  2. Experience in managing substance abuse and substance related disorders

  10. Working knowledge of inpatient and community-based resources for children and adults with mental health and addictions needs.
  11. Must possess a thorough knowledge of the principles and techniques associate with crisis intervention and, recovery based approaches.
  12. Ability to elicit critical information in stressful situations, and provide rapid assessment to formulate an immediate intervention.
  13. Have a strong knowledge of the Mental Health Act and to be able to be a resource on this topic.
  14. Must be able to manage the physical demands associated with responding to an acute crisis in the community (i.e. stairs, attending client’s homes).
