The Algoma Geriatric Network is a network of organizations that provide a comprehensive range of specialized geriatric services to seniors in the Algoma District in partnership with the North-East Specialized Geriatric Services Program.

The Network works toward ensuring that all older adults and their care partners in Algoma have equitable and timely access to high-quality, specialized geriatric services and resources that are part of a coordinated system of care designed to optimize their health and support their independence and quality of life.

Update on your Home and Community Care – Patient Care Plan

On May 31st, 2017, all local home care services and staff from the North East Community Care Access Centre (North East CCAC) transferred to the North East Local Health Integration Network (NE LHIN). 

With this transition, there is no impact to the services you receive and no change to the people providing you with the care you need.

Currently, all patient information booklets are being updated to reflect the NE LHIN organization.  Please know that all phone numbers and office locations referred to in your patient information package stay the same.  You may notice that our patient service staff will answer the phone; “Home and Community Care, North East LHIN.”

We are confident this transition supports improved patient care and helps to ensure patients get the best care possible.
