The Sault Area Hospital Health Records Department is responsible for the collection, storage and retention of your personal health information. We respect and maintain the confidentiality of your records. The Personal Health Information Protection Act (PHIPA) allows you to access health information, which is facilitated through a Release of Information request.

The Health Records office is located on Level 1 and is open Monday to Friday, 8:00 AM to 4:00 PM.

Birth Certificate

The hospital does not issue birth certificates. After birth, the hospital or midwife provides a Statement of Live Birth. This is a permanent identity record, and it is needed to apply for a birth certificate from Service Ontario. To obtain a birth certificate, contact Service Ontario at 1-800-461-2156 or through the Service Ontario website

Death Certificate

The hospital does not issue death certificates. A Medical Certificate of Death is completed by the hospital and is sent to the funeral home with the deceased. A Statement of Death is completed at the funeral home, and the death is registered with the Office of the Registrar General at Service Ontario. To obtain a death certificate, please contact Service Ontario at 1-800-461-2156 or through the Service Ontario website.

If you believe your record of personal health information is not as accurate or complete as necessary, please submit a completed Correction Request Form. This will be to the attention of the Privacy Officer, and please understand that we have 30 days to respond.

There is an administrative fee associated with the request to obtain a copy of your health record. The fee for copying varies with the size of the record and the number of pages copied. Pre-payment of this fee is required, and an estimate can be provided in advance.

Sault Area Hospital no longer releases imaging records via CD. We do provide imaging records to patients and their care providers only through secure, online access. For personal access to your imaging records, please enroll online here, or you can call (705) 759-3689:

  1. Select Option 1 for Patients if you need further assistance or information,
  2. Select Option 2 if you are a Physician, External Hospital or Imaging Clinic,
  3. Select Option 3 if you are calling from a Legal Office or Insurance Company.

Whether you want your records released to a relative, friend, family doctor or another institution, you must submit a signed consent giving the hospital authorization to release your information. 

For patients who are deceased, proof of trustee/executor of the estate, or legal signing authority, must be submitted with your request. Please note that if you have Power of Attorney for Personal Care for a deceased patient, it expires upon death. 

For patients who are incapable of signing a consent, proof of legal signing authority must be provided with the written/signed request for patient information. Proof of legal signing authority such as the Substitute Decision Maker (Public Trustee) may also include the patient’s legal guardian, Power of Attorney for Personal Care, spouse or partner, parent, child, sibling or another relative.
