What We Do
Utilizing an interprofessional team of physicians, nurses and community partners, the Internal Medicine Rapid Access Clinic (IMRAC) provides short-term, timely consultation and stabilization for individuals who are experiencing acute changes or worsening of chronic medical conditions, such as congestive heart failure.
The clinic supports patients who:
- Have recently worsened or newly identified complex medical conditions
- Have concerns that do not require emergency care, but cannot wait for a routine specialist appointment
- Have been recently discharged from the hospital or seen in the Emergency Department and need urgent medical optimization
Community partners include the Group Health Centre and the Community Care Access Centre, ensuring coordinated transitions and continuity of care.
Scope of Care:
While we may see patients with ongoing (chronic) medical conditions, the IMRAC is not intended for long-term or ongoing follow-up.
Our role is to:
- Assess and stabilize your condition
- Provide short-term management over a few visits
- Develop an initial treatment or care plan
Once your condition has stabilized, you will be transitioned back to your primary care provider for ongoing care and follow-up. If you do not currently have a primary care provider, we ask that you arrange future follow-up through a walk-in clinic or virtual care service.

Important to Know:
- IMRAC is not a substitute for a family doctor or long-term specialist care.
- You may be seen for a limited number of visits before being discharged back to your usual care provider.
- Please bring a list of your medications along with any over-the-counter or herbal supplements to your appointment
- Please bring any recent test results to your appointment.
- Volunteers and wheelchairs are available in the main lobby to assist you to the clinic.
