Social Worker (BSW) Mental Health and Addictions Program – Concurrent Disorders Day/Evening Treatment Services – Full Time

The Sault Area Hospital (SAH) is currently recruiting for a Registered Social Worker assigned to the Mental Health and Addictions Program – Concurrent Disorders Day/Evening Treatment Services. The successful candidate will be an integral member of a multidisciplinary mental health and addictions team providing care to those impacted by moderate to severe mental health and addictions challenges.   

 

DUTIES:

  1. Facilitate and score evidence-based bio-psycho-social-spiritual assessments & client experience surveys.
  2. Assist with providing concurrent disorders related support and navigation to ensure individuals remain in the program and ultimately facilitate pathways to other services and transition in care.
  3. Consult within multidisciplinary team regarding evidence based approaches to concurrent disorder treatment and care strategies (i.e. trauma informed care, stages of change, harm reduction, etc).
  4. Educate and support clients and families using well developed evidence-based skills to capitalize and empower clients in their care.
  5. Conduct post-care calls to strengthen continuum of care and transition care plan pathways.
  6. Maintain current knowledge of best practices and benchmarks for the delivery of services, appropriate research, literature, accreditation requirements, policy and legislative requirements and implementing improvements/changes as appropriate
  7. Participate with In-Service programs regarding the social and emotional components of health/illness and their effects on patients and families, as assigned.
  8. Assist in diversion from hospital and the criminal justice system as appropriate.
  9. Work from a continuous improvement framework.
  10. Educate clients, families and other supports on mental health and addictions and how to access support.
  11. Maintain accurate client records and statistical reports as required.
  12. Function as a professional role model demonstrating good interpersonal skills and a commitment to self-growth and development.
  13. Develop and maintain good working relationships with other hospital and community partners while remaining a client advocate.
  14. Participate in peer supervision with other staff, as requested.
  15. Maintain current knowledge of best practices and benchmarks for the delivery of services, appropriate research, literature, accreditation requirements, policy and legislative requirements and implementing improvements/changes as appropriate.
  16. Take initiative to develop and maintain unit policies and procedures, and client centered treatment plans.
  17. Assuming other duties as may be assigned.

 

QUALIFICATIONS:

  1. Bachelor’s degree in Social Work.
  2. Current registration with the Ontario College of Social Workers and Social Service Workers.

 

EXPERIENCE:

  1. Recent experience supporting those with moderate to severe concurrent disorders within a health care environment or community environment
  2. Proficiency in a Microsoft environment including experience in the development of spreadsheets, Word, Power Point, Outlook, and Visio
  3. Skill and experience in bio-psycho-social-spiritual assessment and care and the ability to link clients with the appropriate services are essential.
  4. Human relations skills including empathy, sensitivity, understanding of human and organizational behavior, motivational techniques and counseling skills (e.g. teaching patients).

 

SKILL:

  1. Excellence in problem solving techniques, conflict resolution skills, ability to work effectively on an interdisciplinary team and with the public.
  2. Excellent writing skills including the ability to synthesize information into clear, concise messages both for detailed analytical reports and executive summaries.
  3. Current Basic Cardiac Life Support (BCLS) and First Aid certificate required.
  4. Non-Violent Crisis Intervention Training.

  8. Demonstrates crisis intervention skills for both telephone and person to person situations.
  9. Working knowledge of inpatient and community-based resources for adults with mental health and addictions needs.
  10. Have a strong knowledge of the Mental Health Act and to be able to be a resource on this topic.
  11. Demonstrated behaviour consistent with the Hospital’s mission statement and ICCARE values
