DUTIES:
- Screen and identify patients requiring intervention in assigned patient areas using established nutrition care standards.
- Prioritize and assess/treat/monitor nutrition care of pre-dialysis, dialysis, post-transplant patients in both inpatient and outpatient settings.
- Assess patients’ nutrition needs using medical history, physical assessment, nutrition history, and anthropometric/biochemical data.
- Develop, implement, and evaluate nutrition care plans, including those for high risk patients.
- Provide nutrition education to patients, families, and interdisciplinary team members. Contribute nutrition expertise to interdisciplinary committees, meetings, and initiatives in the department.
- Document according to professional and organizational standards.
- Liaise with community partners as required for continuity of nutrition care.
- Demonstrate behavior consistent with hospital Mission, Values, and Standards of Performance, including demonstrating interpersonal skills and commitment to continuous education and development.
- Practice evidence-based medical nutrition therapy, using current clinical nutrition guidelines to deliver exceptional patient care.
- Maintain current knowledge of best practices and funding criteria for the delivery of services, appropriate research, literature, accreditation requirements, policy and legislative requirements and implementing improvements/changes as appropriate.
- Act as preceptor/mentor to dietetic interns and other students.
- Other duties as assigned.
QUALIFICATIONS:
- Graduate of integrated or post-degree internship or Masters program accredited by the Partnership for Dietetic Education and Practice (PDEP).
- Registration with the College of Dietitians of Ontario in good standing.
SKILLLS:
- Ability to work effectively and efficiently as a team member in a fast paced environment in stressful situations.
- Customer service, interpersonal and telemedicine skills.
- Computer literacy in a Microsoft environment, flexibility to work within different computer program environments.

For clarity, fully vaccinated means that 14 days have elapsed after completion of your vaccine series (i.e. full two doses).
