Occupational Therapist – Program for Assertive Community Treatment (PACT) – Full Time

Sault Area Hospital is currently recruiting for the position of Occupational Therapist in our Program for Assertive Community Treatment (PACT). The successful candidate will be an integral member of the multidisciplinary mental health team and will liaise with the diverse mental health programs within the hospital and community following standards set by the College of Occupational Therapists of Ontario.

DUTIES:

  1. Provide assessment, program planning, treatment, follow-up and program evaluation to clients within the inpatient, outpatient, or community setting.
  2. Analyze and interpret assessment findings.
  3. Generate, implement, evaluate, and treatment plans on an individual or group basis
  4. Provide education and consultation to client and family, caregivers and other health care professionals in the hospital and community to promote optimal level of occupational performance.
  5. Implement strategies to address client safety issues and mitigate risk for critical injury and death.
  6. Educate client and family in coping strategies, decision-making, and utilization of personal and community resources, and make referrals to community resources as required.
  7. Collaborate with clients, families, interprofessional team members, and community partners throughout the therapy process and advocate to ensure client needs are met.
  8. Participate in the discharge planning process to ensure safe, effective, and efficient discharges
  9. Conduct home assessments.
  10. Provide effective written and verbal communication according to hospital and departmental guidelines.
  11. Document assessment findings, analysis, treatment plans, client goals, client’s ongoing progress, effectiveness of treatment plan according to the College of Occupational Therapists of Ontario (COTO) standards and hospital policy.
  12. Research and incorporate best evidence into clinical practice.
  13. Act independently in prioritizing patient needs to ensure individual patients on caseload receive appropriate intervention in a timely manner.
  14. To work in collaboration with a multi disciplinary mental health team and psychiatrist as required
  15. Other duties as assigned

QUALIFICATIONS:

  1. Graduate of an accredited university program in occupational therapy that meets academic and clinical requirements for registration with COTO.
  2. Pass in the Canadian Association of Occupational Therapists national certification exam.
  3. Current registration with the College of Occupational Therapists of Ontario (COTO).
  4. Must maintain current Professional Portfolio and participate in audit process as required by the (COTO).

EXPERIENCE:

  1. Experience working with people who have severe and persistent physical and/or mental illness.

KNOWLEDGE & ABILITIES:

  1. In-depth knowledge of techniques for assessing and treating basic cognitive abilities and activities of daily living.
  2. Strong communication and interpersonal skills to assist in providing relevant care to clients as well as observing and evaluating treatment effects.
  3. Extensive knowledge of community resources.
  4. Ability to work independently as an interdisciplinary team member and interact with clients.
  5. Demonstrated behaviour consistent with the hospital’s Standards of Performance and ICCARE values.

Must have a valid driver’s license, and insurance coverage to allow transportation of clients.
