As part of the provincial government’s public reporting of patient safety indicators, all hospitals with operating rooms are required to publicly report on their compliance with a surgical checklist. This helps to ensure quality and safe surgical services.

The surgical safety checklist is considered to be performed when the designated checklist coordinator confirms that the surgical team members have implemented and/or addressed all of the necessary tasks and items in each of the three phases of the checklist – briefing, time out and debriefing.

The percent compliance is calculated as follows:
