Clinical Learner Orientation Checklist

Please - ensure that you have completed all orientation requirements and modules before completing this checklist. You will be required to submit documentation for two modules: Worker Health and Safety and IPAC Core Competency Training. Please have those certificates available.

To complete your Orientation Checklist, you MUST check (click on) each box to attest to your completion of each module and agree to all attestations.
