Purpose of Position:
Sault Area Hospital is currently recruiting a Patient Flow Supervisor to join our leadership team. Reporting to the Manager, Patient Flow & Registration, you will be responsible for facilitating the timely and appropriate transfer of patients who require admission to the hospital. Working in collaboration with the supervisors, nursing staff, interdisciplinary team and physicians, the Patient Flow Supervisor will assure adherence to established bed flow policies and procedures.
Duties:
- Collaborate daily (or more frequently as required) with the patient care unit leads/supervisors to remain aware of issues impacting bed capacity, including leading and arranging the daily bed meeting.
- Work with unit staff and patient registration to assign optimum bed placement for admitted patients.
- Working with Managers and Supervisors to facilitate inter-service transfers based on patient need.
- Assist in the planning, implementation and evaluation of repatriation processes and practices.
- Ensure the appropriate and timely repatriation of patients from our external partners.
- In collaboration with the Manager, identify trends that impact timely discharges and find solutions for improving patient flow through the continuum of care.
- Identify issues regarding turnaround times, as they apply to the timely moving of patients, and escalate issues that require management follow-up.
- Promote the use and implementation of efficient and effective practices and processes to ensure smooth patient flow.
- Develop plans to support the patient admission process after hours.
- Establish effective partnership with MD’s in facilitating the best placement of patients.
- Collaboratively challenge the unit based interdisciplinary team to identify gaps and delays in patient flow and develop solutions to address them.
- Support initiatives related to efficiency, safety and patient satisfaction as they relate to patient flow.
- Make necessary changes to promote better patient flow.
- Attend unit bullet rounds.
- Support process and service improvement initiatives by identifying opportunities and, leading efforts both with staff and other departments.
- Conduct leader rounding of patients, staff and physicians, providing feedback as required.
- Function as a role model demonstrating good interpersonal skills and a commitment to self-growth and development.
- Support process improvement initiatives and look for opportunities to improve processes.
- Assume any other duties as assigned.
Qualifications:
- Degree or diploma in a regulated health profession with a current registration in good standing with a Regulatory College.
Experience:
- Minimum of three to five years’ recent patient care experience in an acute care hospital setting.
