The Algoma Geriatric Clinic has been providing comprehensive assessments, recommendations and treatment planning for seniors living in the community since 2013. Geriatric Clinic patients are often struggling with declines in ability, falls and/or multiple medical problems.
The Clinic provides specialized geriatric care with goals of increasing functional ability, independence and quality of life for seniors and their care partners.
Treatment is provided by a multi-disciplinary team. Our team is comprised of:
- Geriatricians
- Physicians
- Nurse Practitioner
- Registered Nurse
- Registered Dietitian
- Social Worker
- Physiotherapist
- Occupational Therapist
Referrals to the Algoma Geriatric Clinic are made by a physician or a nurse practitioner. Referrals are reviewed by clinic physicians and staff. After review, an administrative assistant will call the patient or the patient’s care partner to schedule the first appointment. The first appointment a patient has is called a Comprehensive Geriatric Assessment. After this appointment, a patient can expect an appointment with a physician and might have additional appointments with other members of the multi-disciplinary team. Most appointments take place at the Algoma Geriatric Clinic but some appointments may be scheduled in your home.
We ask that our patients always have someone attend their appointments with them. It is important that the person coming to an appointment with the patient know the patient very well.
The Algoma Geriatric Clinic collaborates with other community care partners and the patient’s physician to provide care and help patients achieve goals. Clinic professionals may refer patients or care partners to other support services in the community that may be of benefit. Physicians in the Clinic may also request some additional medical testing to ensure comprehensive treatment.
