Regional Cardiac Care Coordinator (RN)- Cardiac Care- Temporary Full Time

Position Summary

The Regional Cardiac Care Coordinator (RCCC) is an integral member of the Cardiac Care team, responsible for supporting timely access to advanced cardiac services and ensuring high‑quality coordination of care across the patient journey. Working collaboratively with cardiologists, cardiac surgeons, diagnostic departments, and Ontario Health–CorHealth, the RCCC facilitates referrals, monitors patient progress, supports transitions of care, and provides education to patients and families.

This role upholds the principles of Patients First by improving access, informing patients, connecting services, and supporting the sustainability of Ontario’s cardiac care system.

Duties & Responsibilities

1. Care Coordination & Access Management

• Coordinate cardiac referrals and ensure timely processing in accordance with organizational and provincial standards.
• Maintain accurate and complete patient information to support appropriate triage and prioritization.
• Monitor waitlists and patient status, escalating concerns to the clinical team as needed.
• Support coordination of inpatient and outpatient testing, follow‑up appointments, and transitions between care settings.
• Facilitate transfers to other cardiac centres when clinically indicated or when required to support timely access.

2. Patient & Family Support

• Serve as a primary point of contact for patients and families regarding cardiac procedures, timelines, and care pathways.
• Provide education on procedures, preparation requirements, and expectations for recovery.
• Communicate booking details and follow‑up instructions using the patient’s preferred communication method.

3. Collaboration & Interdisciplinary Practice

• Work closely with cardiologists, cardiac surgeons, nursing staff, diagnostic departments, and external partners to support seamless care delivery.
• Participate in patient care planning and contribute to interdisciplinary discussions.
• Liaise with Ontario Health–CorHealth and other regional partners as required.

4. Data Quality & System Stewardship

• Ensure accurate and timely entry of patient information into the Cardiac Registry.
• Participate in data verification, validation, and quality improvement activities.
• Support organizational and provincial initiatives aimed at improving cardiac system performance.

5. Professional Practice

• Provide health teaching to patients and families.
• Maintain current knowledge of best practices, standards, and accreditation requirements.
• Participate in quality improvement, risk management, and professional development activities.
• Adhere to hospital policies, professional standards, and regulatory requirements.

Qualifications

Education & Registration

• Current registration in good standing with the College of Nurses of Ontario.

Experience

• Minimum five years of recent cardiac care experience, including familiarity with cardiac diagnostics and interventions.

Knowledge & Skills

• Strong understanding of cardiac care pathways and patient care requirements.
• Excellent communication, interpersonal, and conflict‑resolution skills.
• Ability to work effectively in a fast‑paced environment and manage competing priorities.
• Proficiency with Microsoft Office and electronic health systems.
• Strong organizational, critical‑thinking, and problem‑solving abilities.
• Demonstrated ability to work collaboratively within an interdisciplinary team.
• Commitment to patient‑centred care and continuous quality improvement
