Geriatric Emergency Medicine
The Geriatric Emergency Management Program (GEM) is designed to provide specialized care for older adults (65+) presenting to the Emergency Department (ED) with concerns related to conditions that commonly affect older individuals, such as dementia, delirium, functional decline, etc. This program is specifically tailored for patients who have the potential to be discharged, aiming to address their unique needs before they leave the hospital to ensure a safe and independent return home.
Patients who consent to participating in our GEM program undergo a comprehensive assessment to evaluate and screen for various factors critical to older adults, including:
- Cognition
- Social Engagement
- Mobility and Falls
- Skin Integrity
- Medication Needs
- Nutrition and Hydration
- Delirium
- Physical Ability
- Continence
- Pain
- Mood and Mental Health
- Sleep
The ultimate goal of Geriatric Emergency Management is to optimize functional outcomes for older adults by offering targeted interventions. These interventions are focused on improving or maintaining independence, reducing the risk of decline, and improving quality of life during and after the hospital stay. The program provides access to appropriate services that maximize the patient’s functional status and improve their overall health outcomes, all with the intention of preparing them for a safe discharge.
