Community Geriatric Rehabilitation
The Community Geriatric Rehabilitation (CGR) program offers specialized geriatric care to help older adults in our community improve their strength, mobility and independence. Guided by a Comprehensive Geriatric Assessment, our team provides patient-centered care focused on enhancing function, promoting independence and improving overall quality of life for the seniors in our community.
Our Interdisciplinary Care Team
The clinic offers a collaborative approach through a specialized team that includes:
- Physiotherapist
- Occupational Therapist
- Registered Dietitian
- Rehabilitative Technician
- Clinical Navigator
What to Expect at Your First Visit
During your first visit, you will be seen by the team for an assessment. Based on the assessment, an individualized care plan and patient goals will be developed. This may include:
- Group exercise programming
- Individual physiotherapy
- Group cognitive classes
- Nutrition counselling
- Referrals to community support services
Caregiver Involvement
We encourage patients to bring a care partner or family member who knows them well to their first visit. Their insight can be valuable in understanding the patient’s history and current function.
