The Algoma Geriatric Clinic provides specialized geriatric assessment and treatment to support older adults in our community. Our goal is to promote independence, improve function, and enhance quality of life for seniors through comprehensive and patient-centered care.

Who Would Benefit from Referral?

Older adults over age 65, who may benefit from a referral include those experiencing:

  • Falls
  • Frailty
  • Functional decline
  • Memory loss
  • Complex medical concerns

Our Interdisciplinary Care Team

The clinic offers a collaborative approach through a specialized team that includes:

  • Geriatricians – physicians who specialize in the care of older adults
  • Registered Nurses
  • Registered Practical Nurse
  • Social Worker
  • Physiotherapist
  • Occupational Therapist
  • Administrative Supports

Referral Process

Referrals must be submitted by a physician or nurse practitioner. After review, the referral will be added to our waitlist. Once the patient has been taken off the waitlist and is ready to proceed, our administrative team will contact the patient or their caregiver to schedule the initial appointment.

What to Expect at Your First Visit

During your first visit, a Comprehensive Geriatric Assessment will be completed by a geriatrician and a geriatric assessor. Based on the assessment, an individualized care plan will be developed. This may include:

  • Additional assessments by team members
  • Medication recommendations
  • Further diagnostics or medical testing
  • Referrals to community support services

We collaborate closely with primary care providers and community partners to ensure integrated, patient-focused care.

Caregiver Involvement

We encourage patients to bring a caregiver or family member who knows them well to each visit. Their insights can be valuable in understanding the patient’s health history and current function.
