Researcher and Research Assistant Orientation Checklist

Please ensure that you have completed all orientation requirements and modules before completing this checklist. You will be required to submit documentation for the following training: Worker Health and Safety, IPAC Core Competency Training and TCPS2: CORE 2022 training certificate. Please have those documents ready. If you completed additional Clinical Trials training, there will be an area for you to submit those certificates. To complete your Orientation Checklist, you MUST check (click on) each box to attest to your completion of each module and agree to all attestations. IF YOUR SUBMISSION IS MISSING ANY INFORMATION, YOU WILL BE ASKED TO COMPLETE THE MISSING INFORMATION AND RESUBMIT THE CHECKLIST.
