Duties:
- Assess patients with severe and persistent mental illness who are at risk of hospitalization or re-hospitalization
- Assist individuals in the transition from an inpatient or emergency department setting to the community
- Consult, liaise and referral to community agencies to facilitate seamless transfer of care
- Liaise with psychiatrist, family physicians /other providers to ensure continuity of care
- Monitor, evaluate and document response and efficacy of psychotropic medications and communicate these results to the primary physician/Psychiatrist
- Provide individual supportive therapy
- Develop and administer group programming for clients with acute mental health issues
- Provide support and education regarding symptom management to individuals and their families
- Work as member of a multi-disciplinary team
- Advocate for clients rights and preferences and assist with system navigation
- Participate in data collection for statistical, Ministry and research purposes
- Act as a direct support to SAH psychiatrists in the management of identified patients
- Willing to be trained with the MCRRT (Mobile Crisis Rapid Response Team) team where you would work in collaboration with City Police to provide acute crisis intervention in the community.
- Coordinate with other community agencies in the provision of necessary services.
- Assessment, psycho education, proactive treatment approach in a supportive and culturally sensitive manner.
- Develop and update client-centered treatment plans.
- Other duties as assigned
Qualifications:
- Current registration with the College of Nurses of Ontario and a current CPR certificate.
- Valid Driver’s License.
Experience:
- Minimum 3 years’ experience in assessing and treating adults with mental illnesses and proven ability to liaise with referral sources, community agencies and other team members to enhance positive patient outcomes.
- Minimum 2 years’ experience planning care in complex situations or for complex client needs based on advanced working knowledge of psychopathology, psychopharmacology, and psychotherapy techniques for individuals, families and groups
- Experience in managing substance abuse and substance related disorders

- Must possess a thorough knowledge of the principles and techniques associated with Crisis Intervention assessment.
- Ability to elicit critical information in stressful situations, and provide rapid assessment to formulate an immediate intervention.
- Must be able to manage the physical demands associated with responding to acute crisis in the community (i.e. stairs, attending clients homes)
- Demonstrated behaviour consistent with the hospital’s iCcare Way
